Zusammenfassung

Scores sind als wichtiges Hilfsmittel zur Kommunikation und Bewertung auch in der Intensivmedizin nicht mehr wegzudenken. Sie reduzieren komplexe Einzelsituationen in Zahlenwerte und ermöglichen so vergleichende Betrachtungen. Sie unterscheiden sich nach Art, Zusammensetzung und Zielpopulation. Die wichtigsten Scores in der Intensivmedizin gehören in die Bereiche Schweregradklassifikation bei Aufnahme (z. B. SAPS), Monitoring der Organfunktion (z. B. SOFA) und Messung des Therapieaufwandes (z. B. TISS). Breite Anwendung finden sie in der Forschung und der Qualitätssicherung, sie werden aber auch zu ökonomischen Zwecken und in der Ausbildung eingesetzt. Scores zur Beschreibung des Schweregrades beinhalten oft auch prognostische Aussagen. Ihre Anwendung bei Therapieentscheidungen sollte aber kritisch gesehen werden. Die Qualität von Scores lässt sich in den Bereichen Validität, Reliabilität, Anwendbarkeit und Relevanz beschreiben und messen.
