Zusammenfassung
Hintergrund
Für das Jahr 2014 erfolgte eine umfangreiche Weiterentwicklung der für die Abbildung von Operationen am Herzen relevanten Klassifikationssysteme für Diagnosen und Prozeduren sowie des German-Diagnosis-Related-Groups(G-DRG)-Fallpauschalenkatalogs. Auch für die Herzchirurgie finden sich für 2014 wieder zahlreiche wichtige Änderungen. Die klassifikatorische Überarbeitung des Operationen- und Prozedurenschlüssel(OPS)-Katalogs ermöglicht im Wesentlichen die spezifische Codierung innovativer Verfahren, die für deren Identifizierung in zukünftigen G-DRG-Kalkulationen von Bedeutung sein werden.
Material und Methode
Es wurde eine Analyse der relevanten Diagnosen, Prozeduren und G-DRG-Strukturen in den Systemversionen 2013 und 2014 anhand der durch das deutsche DRG-Institut (Institut für das Entgeltsystem im Krankenhaus, InEK) sowie das Deutsche Institut für medizinische Dokumentation und Information (DIMDI) publizierten Informationen durchgeführt.
Ergebnisse
Im Rahmen der Anpassung der G-DRG-Strukturen sind besonders die Änderungen der Eingruppierung und damit auch der Erlössituation bei minimal-invasiven Herzklappeneingriffen und sekundären Eingriffen nach Schrittmacher-/Defibrillatorimplantation von besonderer Bedeutung. Eine erneute umfangreiche Überarbeitung der Berücksichtigung von Komorbiditäten in der G-DRG-Zuordnung [“Patient-clinical-complexity-level“(PCCL)-Berechnung] wird zusätzlich zu Erlösveränderungen in der Herzchirurgie führen. Die Auswirkungen der G-DRG-Anpassung 2014 können klinikindividuell je nach Leistungsspektrum variieren.
Schlussfolgerungen
Weiterhin ist die exakte sowie möglichst vollständige Codierung von Diagnosen und Prozeduren in den Kliniken notwendig, um aktuell und auch perspektivisch eine sachgerechte Abbildung insbesondere komplexer Fallkonstellationen zu ermöglichen.
Abstract
Background
The German diagnosis-related groups (G-DRG) system forms the basis for invoicing inpatient hospital services. Cardiac surgery is subject to continuous medical advancements. The correct case allocation in the G-DRG is a major challenge. This paper analyzes and evaluates the relevant developments of the G-DRG system 2014 for cardiac surgery from the medical and classification perspective.
Material and methods
Analysis of relevant diagnoses, medical procedures and G-DRG in the versions 2013 and 2014 was carried out based on the publications of the German DRG Institute (InEK) and the German Institute of Medical Documentation and Information (DIMDI).
Results
A number of coding systems for surgical measures have been newly established or modified: The identification and performance-based mapping of complex and elaborate scenarios were again the focus of the restructuring of the G-DRG system. The G-DRG designation of minimally invasive operations on heart valves and secondary interventions after implantation of pacemakers and defibrillators were the focus of restructuring. The modified calculation of patient complexities and comorbidities in the form of patient clinical complexity level (PCCL) will lead to devaluations. The impact of the changes will have individual variations depending on the scope of hospitals.
Conclusion
A correct and complete coding of diagnoses and medical procedures in hospitals is an essential prerequisite for a correct and performance-based case allocation in the G-DRG system. This applies especially to complex cases.
