Zusammenfassung

Einleitung

Für die Herz-Thorax- und Gefäßchirurgie finden sich im G‑DRG-System 2019 eine Reihe von wichtigen Detailveränderungen. Darüber hinaus können insbesondere die seit dem Jahresanfang geltenden Regelungen der Pflegepersonaluntergrenzen-Verordnung weitreichende Auswirkungen auf die Leistungserbringung der Herzchirurgie haben. Dieser Beitrag beschreibt die wesentlichen Veränderungen zu Dokumentation und Kodierung von Diagnosen und Prozeduren sowie zu Struktur und Bewertung des G‑DRG-Systems 2019 und die Veränderungen der politischen Rahmenbedingungen für die Herz-Thorax- und Gefäßchirurgie. Des Weiteren werden aktuelle Entwicklungen aufgezeigt.

Methodik

Analyse der relevanten Diagnosen, Prozeduren und G‑DRG-Strukturen in den Systemversionen 2018 und 2019 anhand der durch das Institut für das Entgeltsystem (InEK) und das Deutsche Institut für medizinische Dokumentation und Information (DIMDI) publizierten Informationen.

Ergebnisse

Im Rahmen der Anpassung der G‑DRG-Strukturen sind besonders die Abwertungen sog. komplizierender Konstellationen und bestimmter Mehrfacheingriffe aus den G‑DRG F03A und F03B, die Erweiterungen und Differenzierungen der Basis-DRG F08 und F59 für die Bereiche der Herz-Thorax- und Gefäßchirurgie von Bedeutung und werden Erlösveränderungen nach sich ziehen. Die Auswirkungen der G‑DRG-Anpassung 2019 können klinikindividuell je nach Leistungsspektrum variieren.

Schlussfolgerungen

Im Zusammenhang mit der klinischen Dokumentation und Kodierung sollte stets streng darauf geachtet werden, dass die klinische Leistungserbringung zum einen immer eng an den medizinischen Erfordernissen erfolgt, darüber hinaus aber auch immer sichergestellt wird, dass diese medizinischen Erfordernisse aus der klinischen Dokumentation hinreichend begründbar abzulesen sind. Die Auswirkungen des Pflegepersonalstärkungsgesetzes auf die Fachbereiche der Herz-Thorax- und Gefäßchirurgie sind derzeit nur in Ansätzen simulierbar. Hier sind die weiteren Entwicklungen im Jahresverlauf abzuwarten.

Abstract

Background

In the German diagnosis-related groups (G-DRG) system 2019 there are a series of important changes in details for cardiovascular and thoracic surgery. Furthermore, the regulations for the lower limits of nursing personnel ordinance, which came into force at the beginning of the year, could have particularly far-reaching effects on the service provision of heart surgery. This article describes the essential changes to the documentation and coding of diagnoses and procedures as well as to the structure and evaluation of the G‑DRG system 2019. It also describes the changes to the political framework conditions for cardiovascular and thoracic surgery and presents the current developments.

Methods

Analysis of the relevant diagnoses, procedures and G‑DRG structures in the system versions from 2018 and 2019 based on information published by the Institute for the Remuneration System in Hospitals (InEK) and the German Institute for Medical Documentation and Information (DIMDI).

Results

Within the framework of the adaptation of the G‑DRG structures, the devaluation of so-called complicating constellations and certain multiple interventions from G‑DRG F03A and F03B and the extension and differentiation of the basic DRG F08 and F59 are of particular importance for the areas of cardiovascular and thoracic surgery and will result in changes in tarifs. The effects of the adaptation of the G‑DRG 2019 can show individual hospital-specific variations depending on the spectrum of services.

Conclusion

In association with the clinical documentation and coding, strict attention should be paid to the fact that the clinical service provision is always closely linked to the medical requirements but furthermore, it must always be ensured that these medical requirements must also be sufficiently justifiably legible from the clinical documentation. The effect of the nursing personnel reinforcement act on the disciplines of cardiovascular and thoracic surgery can currently only be rudimentarily simulated. In this case the further developments in the course of the coming year must be awaited.
