Das medizinische Versorgungszentrum als ambulantes Klinikportal

Zusammenfassung

Üblicherweise stellen sich Patienten nach Überweisung von ihrem (Fach‑)Arzt in der Klinik zunächst ambulant vor, da die meisten Zuweiser zwar die Notwendigkeit einer Behandlung im Krankenhaus abschätzen können, jedoch i. d. R. nicht beurteilen können, ob die Behandlung dann durch die Klinik ambulant oder stationär, in Vollnarkose oder in örtlicher Betäubung mit oder ohne Anästhesiebegleitung erfolgen wird. Die „Klinikambulanz“ fungiert also für Zuweiser und Patienten als Portal in die stationäre Behandlung im Krankenhaus, wobei aus Kliniksicht im Rahmen des ambulanten Patientenkontaktes die individuelle Therapie bzw. dafür noch benötigte Diagnostik festgelegt wird. Für eine „Klinikambulanz“ stehen in Deutschland verschiedene Organisationsformen zur Verfügung: die Hochschulambulanz, die Ermächtigungsambulanz, die Praxis und das medizinische Versorgungszentrum (MVZ). In diesem Beitrag werden diese Organisationsformen kurz vorgestellt und insbesondere die Vorteile des MVZ hervorgehoben.

Abstract

In general, following referral by their general practitioner or a specialist, patients initially present to the hospital on an outpatient basis. Although most referring physicians are able to correctly evaluate the necessity of treatment in hospital, they are usually not able to assess whether treatment will be on an in- or an outpatient basis, or whether under general or local anesthesia with or without an anesthetist. The outpatient clinic thus functions as a point of entry to inpatient treatment in the hospital, both for patients and for their referring physicians. From the perspective of the hospital, this outpatient contact enables the individual treatment and/or necessary diagnostic procedures to be specified. Various forms of organization are possible for outpatient clinics in Germany: university hospital outpatient department, Ermächtigungsambulanz (hospital-associated institutes for personally authorized outpatient treatment), medical practice, ambulatory health care center (medizinische Versorgungszentren, MVZ). These forms of organization are presented and the advantages of MVZ highlighted.
