Zusammenfassung
Auch wenn die Anzahl von Patienten mit einem Diabetes mellitus kontinuierlich ansteigt, erfährt das Fach der klinischen Diabetologie unter den geänderten gesundheitsökonomischen Rahmenbedingungen in Krankenhäusern in Deutschland kaum noch Bedeutung. Da andererseits die klinische Ausbildung von Ärzten in Krankenhäusern stattfindet, ist das Interesse an dem Fach der Diabetologie gering. Diese Entwicklungen machen es notwendig, neue Versorgungsmodelle zu entwickeln und zu etablieren, bei denen das Fach der Diabetologie im Krankenhaus wieder eine Beachtung findet, um dadurch die Behandlungsqualität zu verbessern. Zusätzlich müssen solche Modelle einen verantwortungsvollen Einsatz von personellen Ressourcen und Verbrauchsmitteln gewährleisten. Der Verbund Katholischer Kliniken Düsseldorf hat mit dem Telemedizinischen Diabetesbetreuungsprogramm (TeDia) ein neues interdisziplinäres und intersektorales Diabetesversorgungssystem etabliert. Dabei werden sämtliche stationäre Patienten mit der Nebendiagnose Diabetes mellitus die in diesen Krankenhäusern versorgt werden, systematisch erfasst, untersucht und während des stationären Aufenthaltes diabetologisch mitbetreut. Durch die Erfassung der Patientendaten in einer elektronischen Fallakte ist eine telemedizinische Betreuung möglich. TeDia wurde im November 2020 durch den Gesundheitspreis des Landes Nordrhein-Westfalen ausgezeichnet.
Abstract
Although the numbers of patients affected by diabetes mellitus are continuously increasing, clinical diabetology does not play any major role in German hospitals due to the changes in the health economic framework. As the basic training of physicians in Germany is primarily located in hospitals, the interest of the training for the subject diabetology is low. Therefore, it is necessary to develop and establish new forms of managed care to improve the treatment quality. Such models of managed care must guarantee a responsible handling of personnel and consumable resources. The Catholic Hospital Group Düsseldorf has established the telediabetological diabetes management system (TeDia), a new interdisciplinary and cross-sectoral diabetes management system. Within this system all patients with the secondary diagnosis of diabetes are systematically detected, examined and diabetologically treated during the hospital stay. All patient data are documented in an electronic patient file and, therefore, a telemedical care is possible. In November 2020 the TeDia was awarded the Heath Prize of the State of North-Rhine/Westphalia.
