Die neue Krankenhausplanung NRW 2022 – Inhalt, Implikationen und Perspektiven für die Gefäßchirurgie

Hohe Krankenhausdichte, Überkapazitäten, Fehlanreize und Diskussion um Qualitätspotenziale haben einen Reformprozess in der Krankenhausplanung in Nordrhein-Westfalen angestoßen.

Ergebnisse

Das Gutachten zur Krankenhauslandschaft NRW legte 2019 strukturelle Defizite mit Kombination aus Über- und Unterversorgung, Erreichbarkeiten, merkantilen Fehlanreizen und fehlenden Qualitätskriterien offen. Das Ministerium für Arbeit, Gesundheit und Soziales als zuständige Planungsbehörde erarbeitete in Zusammenarbeit mit dem Landesausschuss für Krankenhausplanung und etwa 50 Arbeitsgruppen unter Beteiligung der Kostenträger, Ärztekammern und der Krankenhausgesellschaft bis Mitte 2020 eine Arbeitsfassung des neuen Krankenhausplans. Die Fachgesellschaft DGG wurde im Juni 2020 zur Abgabe einer Stellungnahme aufgerufen. Nach abschließender Beratung und Anhörung vor dem Landtagsausschuss trat der Krankenhausplan dann im April 2022 in Kraft. Inhaltlich wurden für die somatische Medizin 30 differenzierte Leistungsbereiche und 60 Leistungsgruppen definiert, die künftig eine sachgerechte und transparente Strukturierung der Versorgung in Verbindung mit klar definierten nachvollziehbaren Vorgaben zur Strukturqualität ermöglichen und die Basis für die Zuteilung des Versorgungsauftrags (Feststellungsbescheid) an das jeweilige Krankenhaus bilden.

Diskussion

Der Krankenhausplan NRW 2022 stellt einen Paradigmenwechsel seitens der Planungsbehörde dar, mit dem künftig eine gestaltende bedarfsadaptierte Krankenhausplanung die bisherige starre Rahmenplanung ersetzt.

Abstract

Background

The high density of hospitals, competition and underutilization of capacities, divergent economic stimuli and discussions on quality adherence have prompted a process of reformation in the state-wide healthcare system in North Rhine-Westphalia (NRW).

Objective

Presentation of the development and the current content of the novel development plan for hospitals in NRW.

Methods

Evaluation and presentation of the report on the hospital landscape NRW, the hospital plan NRW 2022 as well as internal protocols of the task force on hospital planning of the German Society for Vascular Surgery (DGG).

Results

The report on the hospital landscape NRW in 2019 identified structural deficits with a combination of overutilization and underutilization of resources, prolonged transfer times, counterproductive economic drivers and inferior quality standards. The Ministry for Health, Labor and Welfare as the authority responsible for planning, together with the State Committee for Hospital Planning and ca. 50 working groups including healthcare insurance, healthcare providers and representatives of the medical boards, established a draft version of a novel hospital plan in mid-2020. The specialist society DGG was invited to review and comment on the draft version in June 2020. Following a final consensus and a hearing by the State Parliament Committee, the novel hospital plan came into effect in April 2022. For somatic medicine 30 different service areas and 60 service groups were defined that will be assigned to the respective hospital and enable transparent and structured allocation of resources in combination with well-defined requirements of treatment quality.

Conclusion

The hospital plan NRW 2022 enables a paradigm shift of the healthcare authorities from the previous rigid framework plan towards a more flexible, needs-adapted and quality driven hospital plan.
