Zusammenfassung
Die bisherige Aufgliederung und eher stringente Auftrennung der medizinischen Versorgungssektoren (ambulant vs. stationär) befindet sich in einem Umbruch. Aufgrund des enormen Fortschritts im Bereich der arthroskopischen und minimal-invasiven Kniegelenkchirurgie können zunehmend Operationen ambulant erfolgen, die noch vor wenigen Jahren ausschließlich stationär möglich waren. Gleichzeitig nimmt mit wachsendem Verständnis um bedeutende Begleitpathologien (z. B. Beinachsdeformitäten bei Kreuzbandinstabilität) die Eingriffskomplexität zu. Es entscheiden jedoch nicht ausschließlich medizinische Faktoren. Rechtliche Rahmenbedingungen, ungleiche Honorierung sowie Unterschiede bei der Kostenerstattung von Implantaten sind zu berücksichtigen, um komplexe Gelenkchirurgie ansatzweise kostendeckend und damit wirtschaftlich zu betreiben. Der vorliegende Artikel soll einen Überblick darüber geben, welche medizinischen und wirtschaftlichen Überlegungen bei Knieoperationen getroffen werden müssen, um unter Berücksichtigung der aktuellen Rahmenbedingungen eine optimale Patientenversorgung zu gewährleisten.
Abstract
The previous categorization and relatively stringent separation of the medical care sectors (outpatient vs. inpatient) is undergoing a radical change. Due to the enormous progress in the field of arthroscopic and minimally invasive knee joint surgery, operations can increasingly be performed on an outpatient basis that a few years ago were only possible on an inpatient basis. At the same time, the complexity of surgery is increasing with a better understanding of important accompanying pathologies (e.g., leg axis deformities in cruciate ligament instability); however, decisions are not only based on medical factors. Legal frameworks and unequal payment as well as differences in the reimbursement of implants have to be taken into account in order to perform complex joint surgery in a to some extent cost-effective and therefore economical way. This article provides an overview of the medical and economic considerations that must be made in knee surgery in order to ensure optimal patient care and taking the current framework conditions into account.
